To help us prepare for your meeting, you first must be registered in WSSC Water’s Supplier Portal. Please do not submit this questionnaire or required supporting documentation until you confirm you are registered in the supplier portal.

Upon receipt of your completed questionnaire, OSDI will review your submission and
assign it to an OSDI representative. The OSDI representative will contact you to schedule a Microsoft Teams meeting.
